My experience at Clear Choice of Dallas was terrible CHRONOLOGY SUMMARIZED for Tooth 18 by ClearChoice 08-DEC-2014 The 1st tooth 18 was originally received on 08-DEC-2014 broke loose on 07-JAN-2015 and was lost on 18-JAN-2015 26-FEB-2015 The 2nd tooth 18 was received on 26-FEB-2015 29-DEC-2017 The 2nd tooth 18 received on 26-FEB-2015 broke loose on 29-DEC-2017 29-JAN-2018 The 3rd tooth 18 did not fit and could not be used Clear Choice made new molds for 4th replacement tooth 22-FEB-2019 The 4th tooth 18 was received 22-FEB-2018 I had to return the next day to remove extremely sharp metallic edges 02-AUG-2028 The 5th tooth 18 was received on 02-AUG-2018 which appears to be partially porcelain tooth and metal 21-NOV-2018 The 5th tooth 18 was loose the filling was removed and the screw was tightened on 21-NOV-2018 no permanent filling was added and told me to come back later to get a permanent filling 28-JUL-2020 The 5th tooth 18 is loose again sent email and left voice message inquiring whether securing the tooth is covered by a warranty never received a response so I went to another dentist who tightened the loose screw and added a filling for 140 00
